That is symbolic..... to show the connection between the face of Obama and America.
Just as has been pointed out in other posts, on another symbolic observeance, with the 3rd leg. (And NO, I don't think the camera angle is wrong.)
Also if you increase the view to say 400% on the photo, you will see the other arm extended forward on the other side, between his chin and other arm.

It looks like the blonde girl has the word "Europe" or "Europa" on her headband. That and the context of "We Too" on her outfit would seem to suggest that is the case.
